Position Overview: The Clinical Transition Coordinator will play a key role in overseeing the day to functioning’s for the outpatient location under the supervision of the Outpatient Director. The Clinical Transition Coordinators responsibilities include managing inpatient to outpatient RCA step-downs, providing therapy services to clients, fulfilling case management tasks, completing utilization reviews, and performing administrative duties. The Clinical Transition Coordinator is the direct liaison between inpatient and outpatient staff, responsible for working with the teams to coordinate continuation of care from inpatient to outpatient treatment. The Clinical Transition Coordinator will identify appropriate step-downs and educate clients on RCA Outpatient Services. The Clinical Transition Coordinator is actively involved in helping clients and families to identify aftercare needs following inpatient treatment for outpatient treatment services through RCA. This person will operate as a member of the multidisciplinary treatment team by working closely with referral sources, inpatient staff, and the outpatient treatment team. They may also lead group therapy, individual therapy, family therapy, and psychoeducation sessions (PHP, IOP, GOP) as needed. The Clinical Transition Coordinator will inspire and motivate RCA clients to help them develop tools to work toward recovery using evidence-based, trauma informed and-centered practices. Additionally, the Clinical Transition Coordinator will coordinate utilization of client care through reviewing medical records for managed care authorizations and client requests. This individual will act as an advocate for extending client’s treatment to support the clinical Treatment Plan.

Specific Responsibilities:

  • Oversee collaboration and communication with RCA inpatient staff and RCA outpatient staff to assure successful transitions from inpatient to outpatient.
  • Meet with RCA inpatient patients to provide education on RCA outpatient treatment services.
  • Attends inpatient Treatment Team meetings, participates in the weekly inpatient case management meeting and co-facilitates a weekly outpatient education group for inpatient patients.
  • Work collectively with the clinical team to engage, educate, and coordinate client care with the client, their identified supports, and community providers to ensure all services prescribed are implemented.
  • Performs case management tasks to ensure continuation of care and warm handoff from inpatient discharge to outpatient services
  • Actively participate in the development and assessment of individual treatment and service plans.
  • Completes intake Biopsychosocial Assessments, including all electronic forms required by RCA and/or third-party payors.
  • Evaluates clients’ mental and physical health, addiction severity, and/or problem behavior and assess their readiness for treatment and articulates clinical rationale for recommended level of care.
  • Reviews and recommends treatment options with clients, including their families, if willing.
  • Utilizes recognized evidence based therapeutic modalities, such as CBT, DBT, 12-Step, psychoanalytic, motivational interviewing, and stages of change.
  • Performs individual, group, couples and family therapy and education sessions.
  • Is familiar with Medication Assisted Treatment (MAT) and can objectively discuss options with clients.
  • Exhibit knowledge of all continuums of care, including admission assessment and continued stay criteria.
  • Conducts Urine Drug Screens and breathalyzer if necessary.
  • Attends inpatient Treatment Team meetings.
  • Act as an advocate for all clients when working with external providers and speaking on their behalf.
  • Performs Utilization Review tasks, completes utilization reviews, provides information at the request of other members, and communicates all utilization issues that may compromise client’s treatment immediately to treatment team.
  • Provides support to the outpatient team with concurrent review and precertification of services.
  • Consistently works to assist with the development of new and innovative systems/treatment planning ideas to help maximize client days.
  • Works with the treatment team to ensure appropriate utilization of services.
  • Documents all work in the RCA Electronic Health Record, Avatar. Including clinical services, case management services, and all efforts to obtain managed care authorizations.

Education and Experience:

  • Master’s degree in Social Work, Education, Psychology, Mental Health Counseling, or another related human services field required.
  • Licensure as LADC-I, LADC-II, LMHC, LCSW, LPC or LMFT required.
  • Prior experience in a behavioral healthcare or substance abuse settings serving culturally diverse populations required.
  • Experience with Electronic Health Record (EHR) preferred.
  • Excellent verbal and written communication skills required.
  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.
  • Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.
  • Exceptional analytical and problem-solving skills.
  • Proven ability to handle multiple projects under pressure.
  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).
  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the clinical interventions.
  • Understands need for and always maintains appropriate confidentiality when interacting with clients, residents, families, visitors, referral sources and all other contacts.
  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, clients, residents, visitors, families, and referral sources.
  • Knowledge of case management for dual diagnosis clients.

Education and Experience:
* Master’s degree in psychology, social work, or related field is required; Ph.D. preferred
* Strong oral and written communication skills
* Must have licensure either as an LCADC or LPC or LSW or LCSW
* Excellent interpersonal skills
* Proven track record of excellent operations and performance

Work Environment: This job operates in outpatient and inpatient professional office environments. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.

Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.

Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected
